MARRIAGE/CIVIL UNION LICENSE APPLICATION
BY APPOINTMENT ONLY

This office is responsible for the issuance of certified birth, death, marriage certificates and domestic partnership certificates.

To obtain a certified copy of a birth, death, marriage, civil union or domestic partnership certificate, click on the following link and complete the Vital Statistics Request Form.

Once completed, you can print and mail it in with your payment, proper identification and a self addressed stamped envelope.

Fees:

The fees to obtain a certified copy of birth certificates, marriage certificates, domestic partnership certificates and death certificates are set at $25 for the first copy and $5 for each additional copy.

 

APPLYING FOR A MARRIAGE OR CIVIL UNION LICENSE

MARRIAGE OR CIVIL UNION APPLICATIONS ARE TAKEN BY APPOINTMENT ONLY. Appointments are available Monday through Thursday between the hours of 9 a.m. to 3:30 p.m. Please call the Clerk's Office at (856) 988-4429 to schedule your appointment.

PER STATE LAW, APPLICANTS MUST WAIT 72 HOURS AFTER FILING THEIR APPLICATION BEFORE PICKING UP THEIR MARRIAGE/CIVIL UNION LICENSE.

Marriage/Civil Union applications can be taken in the following places:

  • The Township where the Bride/Applicant B resides, or
  • If the Bride/Applicant B is from out-of-state, the Township where the Groom/Applicant A resides, or
  • If both the Bride/Applicant B and Groom/Applicant A are from out-of-state, the Township where the marriage/civil union is going to be  performed. In this case, the license is valid on/in that Township.

Marriage/Civil Union License Requirements

  • Couples must bring with them the following:
  • Drivers license as proof of residency or a Lease with your current address.
  • Witness over the age of 18
  • If available, please bring in one of the following as a second form of identification:
  • Birth certificate
  • Current passport

The following are also needed to complete the application process:

  • Social Security Numbers
  • Names of both sets of parents and the state or country of their birth.
  • Maiden name of both mothers
  • Name and address of person performing the ceremony and the township where the marriage/civil union is taking place.
  • Where there was a previous marriage/civil union, bring divorce papers or death certificate.

If the Bride/Applicant B or the Groom/Applicant A is under the age of 18, but over the age of 16, parental consent by both parents is required. Along with the parents, two witnesses to the signing of the parental consent form are needed at the time of application.

If the Bride/Applicant B or the Groom/Applicant A is under the age of 16, in addition to the requirements listed above, the permission of a County Court Judge is also required.

DOMESTIC PARTNERSHIP CERTIFICATES

Please note, applications are taken by APPOINTMENT ONLY. Please call the Clerk's Office at (856) 988-4429 to schedule.

Requirements for obtaining a Domestic Partnership Certificate in the Township of Evesham are as follows:

  • Applicants do not need to apply in the town in which they live
  • Cash, check or money order in the amount of $28
  • Applicants must execute an Affidavit of Domestic Partnership in the presence of a Notary Public
  • File the notarized Affidavit of Domestic Partnership form with the Local Registrar of Vital Statistics in any municipality in New Jersey to obtain a Certificate of Domestic Partnership.

Applicants must bring with them the following:

  • Current drivers license or photo ID with current address
  • Provide proof of joint financial responsibility as evidenced by one of the following:
    - Joint deed, mortgage agreement or lease
    - Joint bank account
    - Designation of one of the persons as primary beneficiary on the other person’s will
    - Designation of one of the persons as primary beneficiary in the other person’s life insurance policy or retirement plan; or
    - Joint ownership of a motor vehicle
